Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Maintain active state licenses in coverage areas
Maintain board certification
Primary coverage areas include but are not limited to: Springfield MA
Be flexible to travel locally and within licensed states, as needed (up to 25%)
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required
Accommodate field observations and/or “ridealong” experiences, as requested
Maintain all required supplies and associated diagnostic equipment related to DPS offerings
Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
Execute a strong visit completion rate, pacing 35+ completed visits per month

Nurse Practitioner
The Advanced Practice Provider must be a graduate from an accredited School of Nursing program,
Licensed as a registered nurse,
Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP).
The Physician Assistant must be a graduate from a program accredited by the ARC-PA and take the PA National Certifying Exam (PANCE) to use the PAC designation.
PA state licensure required.
NCCPA certification required.
APRN with an unrestricted professional license and DEA, for each state in which they practice.
PA with an unrestricted professional license and DEA for each state in which they practice.
