6 months gap required between for Staff at Program: Aya Program - Providence Health & Services and no current placement allowed at Program: Aya Program - Providence Health & Services.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions.
This means candidates must be responsive and willing to go to a lab to complete health orders as soon as possible after signing.
Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
You must be able to complete labs within 48 hours of signing.
ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any).
Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.
Required Skills/Experience:At least 1 year recent prior experience in vascular ultrasound.
Required Credentials:Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests:RTO must be listed at time of submission.
Unit Details:Neonate, infant, pediatric, adolescent, adult, and geriatric patient population.
Shift & Scheduling:Call required: As needed.
Shift & Scheduling:Weekend requirement: No.
5 days maximum time off
2 Manager/Supervisor references required from within the last 2 years.

RVT (Registered Vascular Technologist (ARDMS))
RVS (Registered Vascular Specialist (CCI))
