Must be able to perform patient assessment, care planning, clinical procedures (including wound care, medication administration, intravenous therapy), monitoring and evaluation, patient education, and coordination of care
Experience providing direct patient care in a home setting
Develop individualized care plans in collaboration with the patient's physician
Educate patients and their families on disease prevention, treatment options, and self-care techniques
Coordinate care with other healthcare providers, community resources, and support services

Physical Therapist
They must have a valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility.
They must have a Master's degree in Physical Therapy; or
They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or
They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
