6 months gap required between for Staff at Program: Aya Program - Providence Health & Services and no current placement allowed at Program: Aya Program - Providence Health & Services.
Pre-employment modules may be required for this role.
All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions.
You must be able to complete labs within 48 hours of signing.
References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
Trauma experience required.
2 years of recent Level 3 or 4 NICU or PICU experience - hard stop.
Strongly desire Level 3 NICU/PICU/Adult ICU experience, but must have level 3 NICU/PICU or adult with strong PICU experience.
RRT (Registered Respiratory Therapist) or the alternative certification CRT
PALS (Pediatric Advanced Life Support)
BLS (Basic Life Support)
ACLS (Advanced Cardiovascular Life Support)
NRP (Neonatal Resuscitation Program – 8th Edition)
Cannot have any hits/suspensions on license or any background hits from any point in the past.
RTO must be listed at time of submission.
Permanent address must be at least 50 miles from facility.
5 days maximum time off
